In 2003, a new nursing role was developed by the American Association of Colleges of Nursing. The clinical nurse leader (CNL) is a master’s prepared nurse with expertise in nursing competencies to ensure delivery of quality care in challenging healthcare environments. CNL’s primary attention is on safety, quality outcomes, evidence-based practice, care coordination, advocacy, and fiscal responsibility, in addition to having expertise knowledge in physical assessment, pathophysiology, and pharmacology. The CNL has a well-rounded background of education to lead in achieving excellence in healthcare. The master’s level education for CNL encompasses nine essential focusses including: science and humanities background, systems and organizational leadership, quality improvement and safety, translation and integration of scholarship into practice, informatics and healthcare technology, health policy and advocacy, interprofessional collaboration, clinical prevention, and master’s-level nursing practice. Following completion of a minimum of 300 clinical hours of immersion learning and practicing in the clinical setting, the CNL is eligible to take the voluntary national CNL certification exam, in which passing the exam allows the use of “CNL” credentials.

The CNL is not an advanced practice registered nurse so does not have prescriptive authority, but the CNL does have the decision making authority to change care plans when necessary. The role is not intended for nursing management positions. The CNL role was designed for key leadership and care coordination with use of a team approach to achieve patient centered care. The CNL role is designed to function in a microsystem setting such as a hospital unit, outpatient setting or home health agency. The CNL is in charge of managing and monitoring client outcomes by following metrics in evidence-based quality improvement and client safety. The CNL is versed in problem-solving, interpretation and management of data, use of critical thinking skills, and the application of social, economic, political, and ethical concepts to patient care problems.

An example of how the CNL influences direct patient care in an outpatient chemotherapy setting is by being utilized as a resource nurse by the chemotherapy infusion nurses and by the oncologist. The CNL can assist with care coordination of patients with complicated regimens. The CNL can perform physical examinations and assess for chemotherapy related toxicities and assist in managing challenging aspects of the oncology patient that may include multiple complex chronic health conditions requiring intervention through collaboration with the Oncologist or APRN. The CNL will oversee the everyday chemotherapy flow of the clinic and assess patients and families to anticipate risks, design and implement plans of care, provide oversight of the care delivery and outcomes for the cancer patient. The CNL can follow data to ensure patients are receiving care for toxicities that are preventable and care to ensure excellence of supportive care in reducing and managing toxicities while delivering holistic care. The CNL could write and update policies for the Oncology Department.
