The Patient in my case study is Shawn. He is a 25 year African American who frequently goes to the clinic. The Patient had been to the clinic four days prior. His complaint was a severe headache treated with a Toradol shot and Ativan. Unfortunately, the Patient is back at the clinic with the same symptoms he had four days prior. Shawn visits the clinic frequently and is a well-known patient at this particular clinic.
An online article defines socioeconomic as “The position of an individual or group on the socioeconomic scale. Combination of social and economic factors such as income, education, occupation, place of residence, ethnic origin or religious background socioeconomic status (Socioeconomic status plays a huge part in every Patient’s well-being)”. My 28-year-old Patient seemed as if she was a vulnerable Patient. This is so because the Patient worried he might not get medication. There are two possible factors why this Patient was worried. The first possible reason could be that he needed insurance or money to pay for the services or pick up his medication from the pharmacy. Secondly, the Patient knew that the healthcare team might consider him as a drug-seeking.
Having an African American background categorizes this Patient as a minority. Culturally, The minority population tends to be poor. In turn, they are not well to do financially and educationally, leading to poverty, “Ethical and racial minorities & socioeconomic status.” As healthcare workers, it is essential to be culturally competent. Therefore, all providers should have the cultural awareness of their patients. “Ultimately, cultural competency is recognized as a necessary means of reducing racial and ethnic disparities in health care. (Becoming a culturally competent health care organization).” Providers should also practice active listening while taking a comprehensive patient history. Accurate collection of health history will lead to correct diagnoses and positive outcomes.
Five questions I would ask the Patient are

2. What brings you to the clinic, and describe your symptoms?
3. on a pain scale of 1-10, how do you rate your pain level
4. What makes the pain better/worse
5. Do you have any allergies to food, medication, or latex?

As healthcare providers, it is imperative that we deliver culturally competent care. This, however, can be very difficult as only 19.4% of nurses are from minority backgrounds (Rosseter, 2022). In order to provide culturally competent care, we healthcare providers must be cognizant of our patient’s heritage, gender, ethnicity, cultural background, and socioeconomic status (Ball et al., 2018). This week we will be considering Shawn Billings case. He is a 28-year-old male who frequently comes to the clinic with migraines. Most recently, he was at the clinic the previous week and four days prior, where he was given Toradol and Ativan. Upon presentation today, he is agitated with an extreme headache. His father also came along, worried his son will not receive any medications since he is a frequent flier.
When building this health history, the provider must consider numerous factors. For one, I would ask Shawn if he has had a work-up related to these frequent migraines and headaches. If not, I would refer him to a neurology specialist regarding his migraines after treating his most current headache. It is also important to ask him if he has insurance and can afford to see the specialist for a workup. Thirdly, I would ask about what he does for a living to see if perhaps that is triggering his migraines. I would also ask if anyone else in his family suffers from migraines, and if so, who. Lastly, I would ask what seems to make the headaches/migraines worse and what makes them better.
The major challenge when dealing with Shawn seems to be the fact that he is a young male who is missing work due to his migraines. Migraines only affect 6% of men but tend to peak between the ages of 25 to 55 years of age, leading to significant time loss from work (Lipton et al., 1997). This could potentially increase Shawn’s financial burdens if he is experiencing them. As a nurse, I can be empathetic, open, and willing to listen to Shawn to see how we can best help him in the long term and not just in this current crisis (Sharifi et al., 2019).
