Sentinel events refer to any patient safety event that results in death, permanent harm, or severe temporary harm that could require an intervention to sustain life (The Joint Commission, 2022). Sentinel events require immediate response and investigation. My clinical setting’s policy on sentinel events entails reporting of the sentinel event, conducting a comprehensive analysis of the issue and investigation, and taking a corrective action plan against the staff involved in the event. My clinical settings’ leadership encourages a culture of accountability and safety among the staff through compliance with the set standards and procedures.

My clinical setting’s policy on sentinel events has a small index of similarity to the Joint Commission’s policy which requires that the affected organization provided support to the patient, the family, and the staff involved, notification of the leadership of the organization, conducting of a comprehensive assessment of the causative factors, identification of corrective actions, a timeline for the implementation of the corrective measures and a systematic improvement of the measurable outcomes.

I think that it is important for the Joint Commission or other regulating agencies to provide policies and procedures regulating sentinel events.  The regulations provide standardization of the reporting process and guidelines. For example, healthcare organizations tend to report events that are standard therefore facilitating the regulatory agencies to apply uniform criteria to investigate and implement the designated corrective plans. Besides uniformity in reporting and implementation of corrective plans, the regulatory agencies promote a culture of safety and accountability among the organizations. By encouraging the health organizations to self-report any a sentinel event, it is an indication that the agencies are encouraging a culture of safety, and are committed to preventing such events in the future.
