An analysis summary word document with one paragraph for your patient/client survey and an additional paragraph for your team survey. Summarize both the patient/client and team results. One paragraph for each. Include:
How many charts were audited (N)
How was the sample generated
Describe the sample using core sociodemographic variables, % (n/N)
Describe the three most notable gaps. Data for each best practice criteria must include % non-compliance. Include the numerator/denominator used to arrive at that %. For example 50% (10/20).
State the focus for any additional follow-up surveys.
Include a third paragraph for ideas and interventions that you may consider to close the gaps

Patient/Client Survey Results
Conducting a patient or client survey can help a nurse to generate insights regarding the respondents’ experience of care and areas that need to be addressed to increase patient satisfaction. 10 charts were audited during analysis. The sample used for the survey was a group of patients receiving anxiety screening and management. From the survey results, 20% of the patients are very dissatisfied with their office experience on the day of the survey. Additionally, 20% do not need therapy and do not find it useful and another 20% find it very hard to start a discussion with the healthcare provider about what worries them most regarding their health. 30% of the participants do not receive provider education on lifestyle changes, 20% do not receive provider education on the importance of psychotherapy in managing anxiety, and 10% of the patients perceive that their provider does not listen to their concerns regarding their anxiety.
Team Survey Results
Surveying healthcare teams provides crucial information regarding their approaches to care. 8 charts were audited during analysis. The sample used for the survey was a team of healthcare providers working with patients suffering from anxiety. From the survey results, 12.5% of the participants are not satisfied with the spirit of teamwork within the organization. 25% do not find their workload reasonable and 25% feel that their direct managers cannot devote time to help when they ask for assistance. Additionally, 37.5% of the participants indicate that they do not have the tools that they need to effectively do their jobs, 25% strongly disagree that their organization demonstrates that employees are an essential part of its success, and 12.5% feel that staff lack a clear and standard method of communication.
Evidence-Based Interventions
The organization should implement evidence-based interventions to close the identified gaps. The evidence-based intervention that can help to close the gaps identified in the patient/client survey are enhancing communication with patients and educating them about their conditions and prescribed interventions (Wieke et al., 2021). To address the gaps identified in the team survey, the organization should train employees to advance their skills, employ adequate staff to reduce the workload, and provide the necessary tools that they require to provide care effectively (Damery et al., 2021).
