Introduction
The nursing profession has changed significantly since its inception. Nurses have become important professionals and have gained more roles in healthcare. However, nurses still face challenges that bar them from performing their duties to their full capacity and ability (Wood, 2010). In 2008, the Robert Wood Johnson Foundation and the Institute of Medicine (IOM) conducted an initiative regarding the future of nursing, for two years. The main purpose of their initiative was to identify ways to respond to and address the needs of nursing care. The IOM initiative had a positive influence on the nursing profession. Furthermore, the IOM report recommended that nurses should have the opportunity to practice to the full level of their education among others (Wood, 2010). The Robert Wood Foundation and the American Association of Retired Persons (AARP) worked in collaboration with one another to develop the Future of Nursing: Campaign for Action. The primary aim of creating the campaign was to enhance, promote, and improve the quality of nursing care in the United States of America (Wood, 2010). This campaign resonates with the aims of the IOM’s Future of Nursing: Leading Change, Advancing Health. The Campaign for Action consisted of 51 action coalitions in all the states as well as the Washing D.C. Various experts are involved in the campaign, such as healthcare providers, educators, policymakers, and business leaders (Hassmiller & Reinhard, 2019). The campaign helps help healthcare stakeholders to address the increasing demand for quality care.
The recommendations of the IOM report cover six different areas including improving access to healthcare services, promoting nursing leadership, and fostering interpersonal collaboration. Other areas include improving the collection of workforce data, as well as the promotion of diversity (Hassmiller & Reinhard, 2019). According to the IOM report, these areas need to be addressed to promote quality of care, safety, and patient outcomes. The 2015 IOM report acknowledged a significant achievement in the nursing workforce. However, the report also admitted that much work still needs to be done and several existing barriers to be eliminated to improve the American healthcare system.
Key Messages Transforming Nursing
Four main key messages structure the IOM Report recommendations. The first key message is that nurses need to practice to the full extent of their education and training. This particular key message recommended in the IOM report empowers advanced practice registered nurses by urging the states and federal authorities to allow the professionals to practice to the full extent of their education (Wood, 2010). Most advanced practice nurses have raised concerns regarding their scope of practice, which they believe has undermined their potential by denying them the right to practice to the ability or extent of their education. The IOM is urging the authorities to improve and update the nurses’ scope of practice. By so doing, they will improve the nursing workforce, patient safety, and quality of care.
The second key message encourages nurses to attain a higher level of education and training. To achieve this recommendation, the government and other relevant stakeholders must improve the education system to promote a seamless academic profession (Hassmiller & Reinhard, 2019). The recommendation will have a positive influence on the American education system to ensure nurses have updated skills, training, and expertise. If they adhere to this recommendation, schools will begin to provide even better education and training.
The third key message advocates for a partnership between nurses, physicians, and other health care professionals. This particular key message promotes collaboration in a healthcare setting as well as effective leadership in healthcare (Hassmiller & Reinhard, 2019). Nurses have a special role to play in healthcare, hence, their contribution must be respected, like physicians and other providers. Developing clear roles can enhance collaboration and efficiency in the workplace. The fourth key message is that better data collection is necessary to ensure effective workforce planning and policymaking (Wood, 2010). Proper data collection promotes effective communication, collaboration, education, and quality care.
State-Based Action Coalitions
A state-based action coalition consists of different individuals collaborating at the state level to enhance nurses’ practice as well as ensure the provision of high-quality care. They can also be described as strong networks of individuals both from the state and national governments who work together to advance and improve health care (Hassmiller & Reinhard, 2019). The primary objective of the state-based coalitions is to discuss issues regarding healthcare structures and processes adopted by the states to create a positive environment for implementing the IOM goals and recommendations (Wood, 2010). The state-based action coalitions promote education, a better workforce, as well as collaboration and partnership in health care.
State-based action coalitions have done significant work in many states across the country. For example, the state-based action coalition in the State of Carolina promotes the use of telehealth in the area to enhance access to health care services (Wood, 2010). Through telehealth, nurses, physicians, and other health care professionals can effectively communicate with patients remotely through video, phones, and other methods (Hassmiller & Reinhard, 2019). The state-based action coalition also promotes collaboration, with over thirty hospitals working together to improve care services in the area.
Barriers to Advancement
There are several barriers to healthcare advancement that need to be addressed for the American healthcare system to be more effective and efficient. All the states are facing at least some barriers to advancement, which include financial barriers, public policy, as well a lack of support to help health care professionals further their education and training (Glover-Thomas, 2020). Economic or financial barriers are a major problem because insufficient funding does not allow for the implementation of quality improvement strategies such as telehealth and others. Also, continued studies allow for the advancement of skills, innovative ideas, and increased knowledge, leading to medical advancement. Denying nurses to further their studies, therefore, hinders advancement (Glover-Thomas, 2020). Pushing for the implementation of policies that promote healthcare advancement can help overcome some of these barriers. For example, advocating for more funds, scholarships, and correction of policies that compromise advancement can be effective remedies.
Conclusion
The Robert Wood Johnson Foundation and the Institute of Medicine (IOM) have played an important role to create a better future for nursing care. The IOM report recommended some key messages whose objective is to promote the quality of nursing care. State-based action coalitions are determined to promote advancement in healthcare. Some of the barriers to healthcare advancement include financial barriers, policy barriers, and a lack of support for healthcare providers to further their studies.
