Assessment Description
In collaboration with the approved course preceptor, students will identify a specific evidence-based topic for the capstone project change proposal. Students should consider the clinical environment in which they are currently employed or have recently worked. The capstone project topic can be a clinical practice problem, an organizational issue, a leadership or quality improvement initiative, or an unmet educational need specific to a patient population or community. The student may also choose to work with an interprofessional collaborative team.
Students should select a topic that aligns to their area of interest as well as the clinical practice setting in which practice hours are completed.
Write a 500-750 word description of your proposed capstone project topic. Include the following:
The problem or issue, intervention, quality initiative, educational need, or collaborative interprofessional team project that will be the focus of the change proposal.
The setting or context in which the problem or issue, intervention, quality initiative, educational need, or collaborative interprofessional team project can be observed.

Diabetes
The Problem
The identified problem is escalating new incidences of type 2 diabetes in the community. The hospital has recorded more than 20 new diabetes diagnoses in the past month as opposed to previous months whereby an average of three people would be diagnosed. There is a concern that the problem might escalate, therefore, an urgent solution is needed to address it (T-Malek & Ahmad, 2022). Doctors, nurses, and other care providers have raised an alarm to relevant authorities regarding the problem. Knowledge regarding the disease is precious in the community at a time like this (T-Malek & Ahmad, 2022). Experts have hinted at a problem in the community that might have led to escalating incidences of diabetes.
Setting
The setting is primary care hospital that serves the entire community. It is one of the biggest hospitals in the area receiving a large number of patients in a day. This hospital provides care to both pediatric and geriatric patients. It offers almost all healthcare services to patients across the ages. Since type 2 diabetes has become a concern to the staff in this facility, there are various programs to aid detection, management, and prevention (Siminerio et al., 2018). Other services offered include urgent care, imaging, laboratory diagnoses, surgery, maternity services, home care, emergency care, and telehealth. This organization is open 24 hours a week and provides patients with quality services regardless of their locations. It has adequate doctors and nurses.
The problem Description
According to T-Malek & Ahmad (2022), diabetes is a global health challenge today that has reached an alarming level. Type 2 diabetes is the most prevalent type of diabetes across the world. About 422 million adults are affected globally (Shirvani et al., 2021). According to Harris (2022), type 2 diabetes mellitus is a chronic illness due to reduces body’s ability to utilize available insulin effectively. This disease is ranked the fifth among major causes of death in Koreans. Type 2 diabetes is commonly caused by unhealthy life habits (Lee et al., 2019). McCay et al. (2019) support this viewpoint by stating that HbA1c may be affected by behaviors in structured diabetes education programs. When one changes behaviors, the level of HbA1c is also impacted. Therapeutic patient education (TPE) is a cost-effective strategy to understand patients and find an appropriate solution (Correia et al., 2022). Type 2 diabetes can have detrimental effects on an individual’s life if not properly managed.
Effects of the Problem
According to Rafie et al. (2021), diabetes causes significant health problems for patients. It is perceived as the leading cause of lower-limb amputation and heart failure in the United States. Gutierrez (2020) states that children with diabetes have a higher risk for adverse effects on their health and education if the condition is poorly managed. Symptoms associated with diabetes include increased thirst, and urination, blurred vision, and tingling sensation caused by persistent elevation of glucose levels in the blood (Harris, 2022). The disease also increases the individuals’ risks of mortality and morbidity. Chronic complications of type 2 diabetes include macro-and-microvascular complications including nephropathy, neuropathy, cerebrovascular disease, cardiovascular diseases, and peripheral vascular diseases.
Significance of the Topic to Nursing Practice
This topic is significant to nursing practice in the sense that diabetes is a major health issue across the world. The nursing practice aims at promoting the health of people at individual, family, and community levels. Prevention and proper management of diabetes are one of the most recognized strategies to improve population health (Siminerio et al., 2018). The information provided under this topic can be used to educate nurses and patients to reduce diabetes incidences and enhance diabetes management practices. This information will help reduce illness and premature death caused by its complications. Therefore, this is an important topic in nursing practice.
