Assessment Description
The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by the Commission on Collegiate Nursing Education (CCNE) and the American Association of Colleges of Nursing (AACN), using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, specific care discipline, and local communities.
This assignment consists of both an interview and a PowerPoint (PPT) presentation.
Assessment/Interview
Select a community of interest in your region. Perform a physical assessment of the community.
Perform a direct assessment of a community of interest using the \”Functional Health Patterns Community Assessment Guide.\”
Interview a community health and public health provider regarding that person\’s role and experiences within the community.
Interview Guidelines
Interviews can take place in-person, by phone, or by Skype.
Develop interview questions to gather information about the role of the provider in the community and the health issues faced by the chosen community.
Complete the \”Provider Interview Acknowledgement Form\” prior to conducting the interview. Submit this document separately in its respective drop box.
Compile key findings from the interview, including the interview questions used, and submit these with the presentation.
PowerPoint Presentation
Create a PowerPoint presentation of 15-20 slides (slide count does not include title and references slide) describing the chosen community interest.
Include the following in your presentation:
Description of community and community boundaries: the people and the geographic, geopolitical, financial, educational level; ethnic and phenomenological features of the community, as well as types of social interactions; common goals and interests; and barriers, and challenges, including any identified social determinates of health.
Summary of community assessment: (a) funding sources and (b) partnerships.
Summary of interview with community health/public health provider.
Identification of an issue that is lacking or an opportunity for health promotion.
A conclusion summarizing your key findings and a discussion of your impressions of the general health of the community.
While APA style is not required for the body of this assignment, solid academic writing is expected, and documentation of sources should be presented using APA format ting guidelines, which can be found in the APA Style Guide, located in the Student Success Center.

Value/Belief Pattern
- What is your name and what position do you hold in this community?
- My name is Mr. Davis Peter, a public health nurse from Phoenix, Arizona.
- What is the predominant ethnic and cultural group in this community?
- We are the dominant white people. Whites make up to about 90% of the population.
- What is your belief as a community regarding health?
- We believe that it is the desire of God for us to be in good health. We believe that illness can occur any time to anyone and that one should seek medical assistance.
- What is the dominant religion in this community? Name some spiritual resources around?
- More than 98% of this community are Christians. We have churches where we worship God every Sunday. We have bibles to read every day to keep out faith and receive encouragement. We have Bible study groups in the community to help in many ways.
- How does your community promote health?
- Yes, this community promotes health in different ways. We have three hospitals around and pharmacies. We also have a church clinic where treatment cost is low. They also provide free medical screening and treatment every Thursday.
- What does the community value? How is this evident?
- We value wealth because we believe once we have wealth, we have almost everything. We the Whites believe in good life and all these are evident from everything you see around. We work most of our time and only little time is left for other activities.
- On what do the community members spend their money? Are funds adequate?
- Most members of this community spend their money on entertainment, expensive alcohol, charity, and food. Yes, there is adequate money because the dominant group here are rich.
Health Perception/Management
- What are the predominant health problems within your community?
- Some of the most common health problems in this community include high blood pressure, diabetes, heart diseases, cancer, and obesity. Stroke is also common among older people. CDC reports 42.2% prevalence of obesity in non-Hispanic White adults. This report is in tandem with what is happening in this community.
- Are children and adult in your community receive proper immunization?
- Yes, they children receive immunization as required. Parents are asked about their children’s immunization status whenever they take them to school, churches, and hospitals. However, adults seem not interested in influenza and other immunizations.
- How many deaths have you had within the last 6 months and what were the causes?
- In the past six months, we have had 2 deaths, one due to cardiovascular diseases and another one due to cancer.
- Does your community have disease prevention programs, such as fitness, dental care, safety and fire drills? Are they sufficient?
- North Scottsdale Community organizes a 5K run/walk once a year, that includes members of North Scottsdale who wish to participate. Participants of these events 2week free handouts regarding healthy lifestyle. These events are not sufficient because they only happen once a year. The gym subscriptions are expensive for people who earn slightly low income. The handouts need educators to take participants though because some of them do not read them.
- Are there referrals made to outside agencies?
- We have almost everything needed to keep us healthy. We have doctors, nurses, and healthcare facilities, including Mayo Clinic. We don’t need to refer to outside agencies unless charity or support organizations. However, one has the right to choose where they want to be treated.
Nutrition/Metabolic
- What is the rate of obesity in your community?
- Not so prevalent but I can see a few people around. I would say quite some are overweight but not obese.
- Do you have programs to provide for those who cannot afford food?
- We have a free food pantry that every Sunday member are able to pick up food such as bread, fruits and canned foods. The food is donated by local grocery stores that are funded by local church group members.
- Do you have vending machine and what is in it?
- We have one vending machine and it is a soda machine.
- Do you have clean water? What can you say about healthy eating in the community?
- Yes, we have clean water all over. Most people eat junk or processes food, at least I cannot deny that. However, we also eat fruits and vegetables.
Elimination (Environmental Health Concerns)
- How does the community ensure clean environment clean and proper waste disposal?
- We have public workers employed by the county authority to cleans the environment and pick up garbage. Residents here are also responsible and are not throwing garbage anyhow.
- Do you have air pollutants around?
- No, we are good, we have no air contaminant around.
- Does the community experience problems of pest and rodents and how do you deal with?
- We had problems with rodents but hired a rodent control to get rid of all of them. It is now one year and we have not experienced any problem.
- Do you share bathrooms or each one has his own?
- Most of people here do not share, but a few individuals do.
- Do you have running water and soap for hand washing?
- Yes, always.
- How do you ensure safety in your community and control proper temperature in the building?
- We check all our alarms are working and temperature programmed for times of services and events
Activity/Exercise
- How does the community encourage fitness among residents?
- We encourage fitness with programs as mentioned earlier.
- What are recreational facilities around?
- We have several gyms around and some swimming pools. We have some stadia where people can go and perform physical activities.
- Do you have health and safety programs?
- We counsel and encouraged our seniors to go to the clinic for checkups and those without insurance to make use of the free clinic.
- Have you had injuries recorded from recreational facilities?
- We have had scraps and strains from children playing at the gym and were treated at the church clinic.
- Can you describe community lifestyle as sedentary or active?
- I would describe as sedentary life because most of them spend a lot of time at work and they use the little time left on computers, watching TVs, and watching movies.
- What means of transport do majority of community members use?
- Most members have private cars and rarely use public means. They also rarely walk to places.
Sleep/Rest
- How many hours do most of the members have to sleep?
- In my interaction with them, many have started less than six hours in a day. They say they believe in work rather than sleeping. Most of them believe that sleep is associated with idleness and poverty.
- What are some of the factors impacting sleep among community members?
- The work shift is a major problem as some work during night hours and sometimes during daytime. Some of them consume stimulants such as caffeine, alcohol, and drugs to keep awake.
- How do you encourage your community members in the areas of sleep and relaxation?
- It’s hard to do anything about how people sleep you can only encourage them to get enough sleep but as a community we hold yearly community healthy sleep campaign where we educate members of proper sleep patterns.
Cognitive/Perceptual
- What is the primary language in your community?
- English is the primary language. Services and programs are conducted using the English language.
- What is the educational level of most members of your community?
- Majority of the members attained bachelor’s degree, and a good number with masters and doctorate degrees. A few have college diplomas.
- What other opportunities or programs do your community offer?
- We as a community contribute money to members who are in colleges and need help funding their college costs. There are several organizations doing this work that we donate to.
Self-Perception/Self-Concept
- What is the median age in the community?
- The median age in my community is 48.9. People aged 18 and above make 71% of the population.
- What are some of the programs and activities related to community building that you carry out in this community?
- Community members participate in charity work by giving donations to respective organizations to assist low income families. Also, members participate in environmental cleanliness, physical activity programs, and health related programs.
Role/Relationship
- How can you describe relationship with one another? Is there friendship among community members?
- Yes, these people love one another and always ready to assist neighbors in case of any problem.
- Are there people you consider vulnerable populations in this community?
- Yes, minority groups like African Americans, Asians, and Native Americans are considered vulnerable. Most of them are low income families, hence, facing challenges with access to food, education, and healthcare. We also have disabled people who are considered vulnerable.
- How does your community relate with the police, fire department?
- We get along well with the police and fire dept. they help us when we have big events
Sexuality/Reproductive
- How do you ensure good relationship within families and among community members?
- The hospitals teach family relationships and relationship with each other.
- How do you educate members against HIV, STD, abstinence and birth control?
- We teach abstinence for our unmarried youth and adult until marriage and based on that birth control is a choice of individual family. We also encourage protected sex and provide free condoms.
Coping/Stress
- What resources are available for your community members when they are in stressful situations?
- We have the hospitals, volunteer care givers, support groups, and organizations that hold counseling sessions for individual and families by appointment. They also do home visits or see members in emergency situations. Members are free to call me, other care givers, support groups, and the organizations anytime for urgent matters.
- How do you deal with members who are victims of crime or criminals themselves?
- We thank God we have the police and authorities. We do not have many crimes around. If a member becomes a victim of crime, we definitely involve law enforcement and we give support.
- Can members get help when in financial need?
- Most definitely, there are organizations that help with true financial need and also give financial teaching for people to better manage their money.
